Reproductive Medicine Associates of New Jersey (RMANJ) understands and acknowledges the need for our patients to have access to their personal medical records. Medical release authorization forms can be accessed and downloaded below. Depending on how you are submitting your request for medical records, please allow at least 1-2 weeks of processing time. Digital delivery like email or fax may take the shortest time, compared to hard copy requests that need to be mailed.
Completed medical release forms may be faxed to 973-290-8370 or dropped off in person at any RMANJ location.

CANCELLATION POLICY
As you know, waiting for an appointment to begin fertility treatment can be stressful, so we ask you to be considerate of other patients who are waiting for their appointment with RMANJ. To ensure every patient is satisfied, here at RMANJ, we have developed guidelines for canceling and rescheduling new patient appointments. Please review the following and call the patient liaison department at 973-656-2089 with any questions: If it becomes necessary for you to cancel your appointment, we require at least 72 hour notice, so that we can contact other patients who may be able to take your scheduled appointment. If you do not contact us, RMANJ can bill you for the missed visit.
CANCELLATION RESCHEDULE POLICY
If you have cancelled or rescheduled your appointment under 72 hours, RMANJ will ask you to complete a Credit Card Authorization Form for payment. This will ensure us of your commitment to your next rescheduled visit. You will be charged $250.00 fee for the initial consultation fee and when you come in for your consultation, you will be reimbursed this fee minus your co-pay.

POLICY REGARDING AGE AND INFERTILITY TREATMENT

One of the most challenging aspects of the infertility experience is dealing with the emotions related to medical treatment, the uncertainty about outcomes, and the challenge of having to make important decisions. Infertility can be one of the most difficult life events that an individual or couple experiences. Coping with making many intense medical decisions and the uncertainty that infertility brings can create an emotional upheaval for many, and that is where the team at our fertility clinic in NJ can help.
Our fertility doctors and Wellness Team are here to help you learn how to cope with the physical and emotional challenges associated with infertility. For some, infertility can be physically and emotionally painful, an intrusive process. Others need help focusing on how to deal with another’s responses, how to choose the right medical treatment, or how to begin exploring other family building options. For others, it may be on how to control, stress, anxiety, or depression.
Counseling & Support Options at Our Fertility Clinic
Jennifer has worked as a pediatric nurse at St. Barnabas Medical Center and a former IVF Nurse at RMANJ. She completed her Bachelor of Science in Nursing at Villanova University, then went on to pursue a Master of Social Work at Columbia University. Jennifer currently runs a private psychotherapy practice in Morristown, NJ and serves as RMA’s Director of Mental Health. She is an active member in the Mental Health Professionals Group of ASRM and was the previous chair of the mental health mentorship committee. Jennifer brings a unique combination of experiences in nursing, social work, infertility and adoption to the practice.
Lauren is a Counselor with the Complementary Care mental health team at RMANJ. She earned her master’s degree in Professional Counseling from Seton Hall University. Her work focuses on supporting individuals and couples in infertility treatment. Prior to working at RMANJ, Lauren spent two years with The New Brunswick Counseling Center providing counseling for individuals with substance abuse issues. Her passion for assisting those who need assistance in daily life brought her to RMANJ in July, 2015. Lauren is a member of the Mental Health Professional Group of the American Society of Reproductive Medicine.
Jackie earned her master’s degree from the University of North Carolina, Charlotte, and has ten years of experience working in the field of mental health. Jackie has been involved in the field of infertility counseling since 2014 and also has a private practice in Summit, NJ. The focus of her work is on general infertility counseling and supporting individuals and couples through 3rd party reproduction. Prior to joining RMANJ, Jackie has worked in private practice for five years and in college mental health for seven years. Jackie is a member of ASRM and RESOLVE, where she facilitated support groups around conceiving through donor egg.
Telehealth diet and nutrition appointments available.
Numerous studies have shown that specific changes to one’s diet can improve fertility, prevent recurrent miscarriage and support a healthy pregnancy. A healthy and balanced diet can play an important role in fertility and pregnancy, particularly with the management of Polycystic Ovary Syndrome (PCOS). Our registered dietitians work closely with your Clinical Care Team to provide personalized nutrition counseling and dietary education classes in each of our RMANJ office locations.
At RMANJ our comprehensive health and nutrition program embraces the importance of proper diet and nutrition:

Desiree Martin has a Bachelor of Science in Nutrition and Dietetics from West Chester University in Pennsylvania where she graduated with summa cum laude honors. Desiree became a registered dietitian in 2014 and began working as a clinical dietitian at CentraState Medical Center in Freehold shortly after.
Her approach is to work with the patient to create an individualized meal plan and balanced lifestyle that accommodates the patient’s specific nutrition goals using scientific and evidence based information and education tools.
She sees patients in RMANJ’s Freehold, Eatontown, and Hamilton offices.
Leda Ristovic holds Bachelor & Master of Science degrees in Nutrition/Dietetics, from Montclair State University. She became a registered dietitian upon successfully completing 1200 hours of supervised practice with Iowa State University’s Dietetic Internship program.
Ristovic also holds a certificate of training in adult weight management, obtained a four part breastfeeding training, and is a certified food safety manager. Her ultimate goal in her career is to educate others on how to make healthier choices and lead balanced lives, using scientific and evidence based information.
Ristovic sees patients in RMANJ’s Englewood, West Orange, and Morristown offices.
Gabrielle Rutenberg holds a Bachelor’s Degree in Nutrition with a concentration in Dietetics from Montclair State University. She successfully completed her dietetic internship with the Sodexo NY/Metropolitan Area internship program, which consisted of 1200 hours of supervised practice with an emphasis in medical nutrition therapy.
She is also a member of the Academy of Nutrition and Dietetics. Her goal is to educate and empower her patients to make healthier food choices. She believes in a whole food nutrition approach and that we can heal ourselves and reach optimum health with healthy diet / lifestyle change.
She sees patients in RMANJ’s Basking Ridge, Somerset, and Springfield offices.
Acupuncture can increase fertility by reducing stress, increasing blood flow to the reproductive organs and balancing the endocrine system. In addition to traditional acupuncture, we also offer needle-free laser acupuncture based on the same principles.
In a large prospective randomized controlled study conducted at RMA, laser acupuncture treatment performed pre-and-post embryo transfer during an IVF cycle resulted in a significant improvement in implantation rates compared to traditional needle acupuncture.
Download ResearchAt RMANJ, treatment with laser acupuncture costs $225 and can be conveniently scheduled before and after embryo transfer. Payment is required at time of service, some patients may be able to seek insurance reimbursement.
Traditional acupuncture at RMANJ costs $175 for an initial consultation which includes treatment, with follow-up sessions at $100 each session. Many patients prefer to purchase multiple sessions at a discounted rate, $360 for four treatment sessions, $850 for ten treatment sessions.
Carolyn received her Diplomate in acupuncture from the Eastern School of Acupuncture and Traditional Chinese Medicine in Montclair, NJ and specializes in fertility and reproductive medicine. She is a member of the NJ Acupuncture Association and has been practicing for over 8 years.

Fertility monitoring allows our physicians to monitor how a patient is responding to fertility treatments. Morning activities normally include an ultrasound and bloodwork. Your physician will review the results of your progress each day and provide you with feedback and instructions for a continued successful path to parenthood.
